Re: Incident Report of March 18, 2006
Dear Kevin:
This is to advise you of the following incident report which occurred on March 18, 2006, during the 3rd Annual Cardile Bros. Mushroom Co. Delaware State Doubles Championship, held at the Kirkwood Fitness and Racquetball Club located in Wilmington, Delaware, as described further below.
While playing a tie-breaker game in the Men’s B Doubles’ division match, Mr. Ernie Lehman (“Mr. Lehman”) collapsed on the court briefly loosing consciousness. Immediately thereafter, two fellow DARA players, Dr. Steve Dellose, MD (“Dr. Dellose”) and Mr. Daniel Weghofer (“Mr. Weghofer”) voluntarily rendered medical assistance to Lehman. They applied mouth-to-mouth resuscitation and Automatic Electric Defibrillator (“AED”) in an effort to help Mr. Lehman regain consciousness.
At the same time, “911” assistance was called, and the New Castle County paramedics among other emergency personnel, arrived at the scene within 7-9 minutes.
Approximately 12-15 minutes later, with the professional efforts of New Castle County emergency personnel Mr. Lehman regained his consciousness. Next, emergency personnel transferred Mr. Lehman onto a mobile gurney, and he was immediately transported to the nearby St. Francis Hospital in Wilmington, Delaware.
Later on that day, we were advised that Mr. Lehman suffered a heart attack, and a catheterization medical procedure had been performed. At this point, it is our understanding that Mr. Lehman is in stable condition and is currently scheduled for bypass surgery on March 21, 2006.
